Quantitative MRI Biomarkers for Differentiating Low-Grade and High-Grade Gliomas: A Prospective Study

Abstract

Objective: To evaluate quantitative MRI biomarkers that differentiate low-grade from high-grade gliomas using diffusion and perfusion imaging.
Methods: Fifty patients with histologically confirmed gliomas underwent diffusion-weighted and dynamic susceptibility contrast MRI. Apparent diffusion coefficient (ADC) and relative cerebral blood volume (rCBV) values were analyzed.

Results: Mean ADC was significantly lower in high-grade tumors (0.89 ± 0.12 ×10³ mm²/s) than in low-grade (1.22 ± 0.16 ×10³ mm²/s, p < 0.001). Mean rCBV was higher in high-grade lesions (4.1 ± 0.9 vs. 1.8 ± 0.5, p < 0.001).

Conclusion: Combined ADC and rCBV metrics provide accurate noninvasive differentiation of glioma grades and may reduce the need for early biopsy in ambiguous cases.
